Door lock screws backed out on a busy Friday afternoon

Last week in a 12-story office building near downtown Austin, I had a car get stuck between floors because the door lock contact screws vibrated loose. Took me 45 minutes to find it since the controller showed a random fault code. Anyone else run into this on older Otis units?

Hate when that happens, especially on a busy Friday when you just want to get home. Had a similar issue on an old Otis 211 at a mall a couple years back. The car kept jumping floors and the door lock screws were just a hair loose too. Took me forever to track it down because the controller just threw a generic "door open" fault.

Could a loose wire somewhere cause that same generic fault code too?
