A client asked me to 'just do the plumbing' on a drawing that was basically a cartoon.

They sent over a sketch on a napkin, literally, with a note saying 'make it work' and a budget of $500. I spent two hours just explaining why we need a real plan before I can even give a quote. How do you guys handle it when a client thinks your job is just the easy part at the end?

Napkin sketches are a classic. But two hours to explain basic planning? That's a lot of free consulting. A real plan isn't just paperwork, it's the only way to know what "make it work" even costs. Next time, maybe send a one-page list of what you need from them to start. If they balk, the problem solves itself.
