The spare battery mystery that took me 5 months to solve

My headlamp kept dying halfway through evening job site walkthroughs, so I bought a spare battery pack and swapped it nightly, but it still failed around the 40 minute mark. After 5 months I finally checked the charger itself with a voltmeter and found it was only putting out 60% of its rated output, so both packs were getting undercharged all along. Has anyone else had a charger quietly fail like that and drive them crazy before they checked the obvious thing?
