The domino cabinet door that almost took out my router table.

Was fitting doors on a job over in Salem and one domino joint was slightly off, so the whole panel twisted when I clamped it. Spent a whole Saturday making a new one and realized my fence was out of square by 1/16th, not the domino at all. Anyone else chase a setup mistake for hours only to find it was a stupid simple thing?
