Representation mismatch tanked my SSDI hearing prep

My lawyer kept saying my claim was solid but never asked for my work history details. I was a mechanic in Fresno for 18 years and he wrote I did data entry. Got a copy of the file last Tuesday, big red flag. Anyone else caught mistakes like this before a hearing?
