Contractor just told me his mix design was off by 10%, I was floored

Got a call from a supplier last week about a job we poured back in March. He said the mix we used was actually supposed to have 10% more fly ash in it, the paperwork got misread at the plant. We had some cold joints and a few surface cracks come up in the last month or so, and now I'm wondering if that's why. I went back and checked our pump schedule and the cure time we gave it, everything else was textbook. But that 10% difference could explain why the slab in the loading dock is wearing faster than we predicted. Has anyone else had a plant mix screwup show up months later, or am I just unlucky here?
