Just realized my CRM was logging fake meeting notes from last month

I was digging through our HubSpot records to prep for a quarterly review with a client in Dallas and found 12 entries from our junior rep that were clearly copy-pasted from templates. The times didn't match the calendar, the action items were identical word for word. Had to fire the guy on Tuesday and now I'm manually re-entering 3 weeks of real notes. Has anyone else caught their sales team faking activity logs in your reporting tools?
